If you are a patient and travelling abroad you may need vaccinations. To find out what is required for the area you are traveling to, please either submit the below form or print off, and complete the Travel Risk Vaccination Form and drop off at the reception desk. Once this form has been received, someone form the surgery will contact you within 48 hours with details of any vaccinations that are recommended and an appointment made if necessary. Please note, some vaccinations require two or three courses before you are fully covered. Therefore, please give approx 8 weeks to return your forms for assessment before you travel.
Please note also We are not a Yellow Fever Centre.
The NHS supply basic vaccinations which are Tetanus, Hepatitis A, Typhoid, MMR, Polio and you will need to consult with a travel clinic for private vaccines/malaria tablets or to discuss complicated itineraries.
